HealthCheck 

HealthCheck is a free health program for children under age 21 who have Medicaid. 

It provides checkups for your children to find and treat any health problems early.

A referral by your doctor may make your children eligible for some treatments not normally paid for by Wisconsin’s Medicaid program, as long as they are medically necessary.

HealthCheck covers the care for any health problems found during the checkup.

If HealthCheck is requested for a child who is one year or younger who is due for a checkup, the HMO must schedule the checkup within 30 days of the request.

If HealthCheck is requested for a child who is one year or older who is due for a checkup, the HMO must schedule the checkup within 60 days of the request.

HealthCheck Schedule 

One (1) checkup at:  2 weeks 2 months  4 months  6 months  9 months  12 months 15 months one checkup every year after that, up to age 21.

 
A HealthCheck includes: 

a health history blood and urine lab tests complete immunizations (shots) physical exam developmental assessment hearing and vision test lead tests
