Family
Planning
You may go to any family
planning clinic inside or outside your HMO that will accept your Medicaid
card. However, HMOs encourage you to use your HMO doctor because he or
she can better coordinate all your health care.
Dental Services
If your HMO provides dental
services, then you must pick a dentist from a list that is provided to
you.
If your HMO does not provide
dental services, you may get dental services from any dentist who will
accept your Medicaid card.
Chiropractic Services
If your HMO provides chiropractic
services, then you must pick a chiropractor from a list provided to you.
If your HMO does not provide
chiropractic services, you may get services from any chiropractor that
will accept your Medicaid card.
Transplant Surgery
HMOs pay for cornea and
kidney transplants and will cover the “work-up” and pre-surgery expenses
for bone marrow, liver, heart, heart-lung, lung and pancreas transplants.
The transplant surgery and follow-up care will be covered by fee-for-service
Medicaid.
Transportation
Medicaid-HMOs are required
to provide transportation to your medical appointments. Ask your HMO how
you can get a ride to and from your doctor’s appointments or hospital visits.
Many Milwaukee HMOs provide door-to-door taxi service.
School Based Service (SBS)
School based services are
a benefit paid for by the Wisconsin Medicaid program for all Medicaid and
Medicaid-HMO recipients who are attending school. This benefit includes
services like occupational, physical and speech therapies.
WIC Program
Good nutrition is important
to a healthy child. WIC is a special supplemental food program for pregnant
women, infants and children. It helps pregnant women and children have
a normal, healthy nutritional level. WIC agencies and Medicaid-HMOs work
together to provide services to eligible recipients.
Telephone Assistance Programs
It is important to have
a telephone in order to get health care when you need it. Ameritech gives
a $5.25 credit per phone bill and offers two telephone assistance programs
for customers who receive Medicaid. If you currently have telephone service
and think you may be eligible for one or more of these programs call 800-924-1500.
If you currently do not have telephone service you should call 800-924-1000
to apply for the service. Ameritech will verify your Medicaid eligibility
with the State of Wisconsin for the following two programs.
Usage Assistance Credit (UAC)
-UAC provides Ameritech customers with a one-cent credit per call for every
call you make above 150 calls, up to a maximum of 1,200 calls.
Telephone Assistance Grant
(TAG) TAG allows Ameritech to waive a customer’s installation fee.
